Outpatient Services
The multidisciplinary team comprised of a clinical manager, psychiatrist, psychiatric nurse practitioner and licensed clinicians coordinates care and meets with the patient both in groups and individually. Treatment is designed to assist patients in developing a plan for long-term recovery.
Each patient receives a comprehensive interdisciplinary evaluation before being assigned their own treatment team. The team develops a personalized, problem-specific treatment plan that is updated weekly. This plan may involve provider meetings, case manager meetings and family contact.
Discharge planning begins at the point of admission and involves collaboration between patient, family and collateral supports to develop a plan for continuing improvement. We also coordinate with the primary care physician, outpatient providers and collateral contacts.
Partial Hospitalization Program (PHP)
Full-time day treatment is appropriate for adults, ages 18 and over, who would benefit from an outpatient program. The program is designed to help prevent hospitalization or to facilitate the transition from the hospital back into the community.
Intensive Outpatient Program (IOP)
This program is for adults with a behavioral health issue who are stable enough to live at home during treatment. Designed to support patients as they develop positive coping skills, it can serve as a bridge between acute care and individual therapy. Part-time day treatment can be used as standalone care or a step-down from an inpatient program.
